QI 1268 Improving the Documentation of Handover at Brooklands Hospital

Abstract
Aim: Handover documents to be correctly completed 100% of the time on the units at Brooklands Following a CQC visit in 2023, units at Brooklands were unable to evidence patient handovers taking place. Units across the Brooklands site developed a handover form and reviewed this so a more simplified form was available to capture essential information and avoid duplication. A Standard Operating Procedure (SOP) was developed to accompany this document. A clinical audit of handover forms showed that documentation of handover was still poor after implementing the new form, and it was identified that further work was needed to make improvements to practice. Further change ideas included developing a pre-populated crib sheet of a ‘good’ handover example, and adding and rewording some sections of the form. Project Impact: Handover forms are now completed consistently for patients at Brooklands. There have been improvements in completion for all sections of the handover.
