Comparison of Post-Operative Complications of Ileostomy Versus Primary Repair in Tuberculous Perforation of Small Bowel

Abstract
Introduction: The study aims to compare outcomes of primary repair and ileostomy formation in patients with spontaneous intestinal tuberculous perforation.
Method: The study was conducted between April to October 2010 at a tertiary teaching hospital. Sixty patients with intestinal tubercular perforation were included in the study and randomly allocated into two groups. Group A consisted of 30 patients who underwent primary repair. Group B consisted of 30 patients and underwent ileostomy formation., The two groups were compared with respect to outcomes including complications (peritonitis, wound infection, fever, obstruction fistula formation) during the inpatient admission, and complications observed at the second and fourth week follow-up.
Result: In Group B, 30% of patients had complications during the inpatient admission compared to 40 % in Group A. At week two, 6.6% of patients in group B had complications compared to 16.6% in group A. At week four, 6.6% of patients in Group B had complications compared to 13.3 % in Group A.
Conclusion: In our study, patients who underwent ileostomy for tubercular perforation had less post-operative complication rates in both inpatient and outpatient settings.
