Abstract
Project Aim: To increase the completion rate of outcomes from MDT discussions on Amber Unit for Dr Roy’s patients to 80%.
It was noted during weekly MDTs on Amber Unit that previously discussed outcomes were not consistently completed in time for the next MDT, and communication of outcomes between different members of the MDT could be challenging due to staff accessing the ward round using different methods. This miscommunication had potential to cause delays to patient care and impact patient experience, limit progression and extend length of stay.

Project Impact: While the outcome completion rate did increase from 54% to 78%, the team were unable to consistently meet the 80% target. It is therefore recommended to hand over to new resident doctors, make alterations to the initial change idea and run further PDSA Cycles until the aim is consistently achieved. Data should be continually monitored on an SPC Chart and act accordingly if the completion rate drops.
