Abstract
Project Aim: To increase the number of submissions of DASH forms from mental health wards from an average of 1 per month in August 2024 to an average of 3 per month by January 2026.
The Safeguarding Team completed an audit of Domestic Abuse, Stalking, Harassment and Honour Based Violence Assessment (DASH) forms submitted by wards during the 6-month period 01/10/23-31/03/24. Only 6 DASH forms were submitted by MH wards during this period which is significantly less than would be expected. The team completed questionnaires with ward managers to identify current knowledge, barriers, support required and ideas for improvement. These formed the basis of a driver diagram. Change ideas were tested over 6 months and data on the
number of DASH forms submitted, contacts for advice and staff feedback were collected to measure progress over time.

Project Impact: The average number of DASH submissions has increased from 1 to 2.4 per month. There has also been an increase in the number of contacts relating to DA from ward staff from an average of 1.6 per month to 3.5. The Safeguarding Team’s presence on the wards is now much stronger. Staff are more familiar with the team, more connected, and more willing to start conversations.
