QI 1323 Children's Physical Health MDT Single Point of Access

Abstract
Project Aim: To improve the MDT referral pathway to reduce referral times for CYP new to services with complex physical health needs. Children and Young People with complex physical health needs require input from multiple community services and experience lengthy assessments. On initial referral, this frequently leads to duplication, cascading of referrals and therefore delays in accessing all required services at the outset. Many patients may require additional support when new to navigating health services, especially those who are newly arrived in the UK and may have limited understanding of the NHS and medications, for example.
