Abstract
The problem or issue we were looking to fix and why it’s important:​ ​To improve the care of patients who have an inevitable death whilst admitted at UHB, ensuring timely CPR decisions, reducing unnecessary/futile interventions and prioritising a holistic approach​ UHB Patient Safety Investigation Response Group (PSIRG) identified that patients have been resuscitated against agreed patient plans and wishes​ ​UHB Trust has higher than the national average rate for cardiac arrests however most deaths in hospital are ‘ predictable’ National Cardiac Arrest Audit (NCAA 2024)​ ​UHB 33% of patients that survive a cardiac arrest have a DNACPR/ CPR not recommended decision put in place at the time of the event after Return of Spontaneous Circulation (ROSC)
